# How to write a SCAR suppliers actually answer

What a supplier corrective action request should contain, how to judge the root cause and corrective action you get back, and how to stop chasing late SCARs.

FactoryQA quality teamOctober 25, 2026

A supplier corrective action request (SCAR) asks a supplier to contain a problem, find its root cause and fix it for good. Many SCARs come back late, vague or not at all. Most of the time the request itself is the problem.

*1***Issue found**Facts and evidence

*2***SCAR sent**With a due date

*3***Containment**Within days

*4***Root cause**5 Whys, fishbone, 8D

*5***Corrective action**Process or controls changed

*6***Verified**Next lots checked

Closed only when the fix is proven

## What a good SCAR contains

- **The facts.** Part number, revision, lot or heat number, quantity received, quantity rejected, and where it was found (receiving, in process, at your customer).
- **The requirement and the result.** The drawing callout or specification, and what was actually measured or seen. Photos and inspection data remove arguments.
- **What you need and by when.** Containment within a few days (stock at the supplier, in transit and at your site), then root cause and corrective action by a stated date.
- **How you will judge the answer.** Say you expect a root cause method such as 5 Whys, fishbone or 8D, and evidence that the action works.

## Judging the response

Reject answers that blame the operator and stop at retraining, or that describe the containment as the corrective action. A good response shows the root cause was verified, explains why the existing controls missed it, changes the process or the controls, and says how effectiveness will be checked. Look for the same fix applied to similar parts and processes.

## Close the loop

- **Verify.** Check the next lots more closely before closing the SCAR.
- **Link it.** One root cause often explains several NCRs; link them so the history is complete.
- **Score it.** Response time and repeat issues belong in the supplier's scorecard.

## Stop chasing

Email SCARs get lost in inboxes on both sides. Track every SCAR with its due date and owner, send reminders automatically, and escalate when a response is overdue. Make it easy for the supplier to respond: a link to a form beats a Word template attached to an email.

Supplier Quality in FactoryQA: SCARs, scorecards and certificates, with a portal suppliers answer through.

In FactoryQA you send a SCAR straight from the supplier issue. The supplier responds through the portal with containment, root cause and corrective action, without a login. 30-day reminders and 35-day follow-up tasks keep responses moving, and the result feeds the supplier's scorecard.
